Doc is back, and this premiere immediately throws us into a deeply emotional, very messy, and frankly fascinating new chapter for Amy Larsen. There is so much happening from the start: new interns, a troubling medical mystery involving a group of students, Amy’s ongoing recovery, lingering tension with Jake, and the arrival of Blair Underwood’s Dr. Richard Grant. It is a lot—but in the best, very Doc way.
The episode opens with a student performance of Romeo and Juliet, which is admittedly a little confusing at first if, like me, Shakespearean language is not exactly your strongest subject. I love the concept of the plays; I just occasionally need someone to translate what everyone is saying into modern English. Still, the opening quickly establishes that this production is far more than a cute school-theater moment. One of the students, Tori, begins having what appears to be a seizure, setting off the central medical case of the episode.
And because this is Doc, the medical mystery is never just a medical mystery. It is also about fear, memory, trauma, and the things people carry with them long after everyone else assumes they should be “fine.”
Amy’s Return Is Hopeful and Scary
The premiere wastes no time reminding us that Amy’s recovery remains the emotional center of the series. She is undergoing brain imaging to assess her recovery, and there is encouraging news: her brain is healing. That is genuinely wonderful, especially when we think about everything Amy has lost. Seven years of memories are gone. Her relationships, career, family dynamics, and sense of self have all been altered by something she had no control over.
The possibility that she could recover more of herself is hopeful. But Amy’s return to work also comes with huge questions.
There is something inherently uneasy about the people closest to Amy—Michael, Gina, and the doctors who work with her every day—also being the people tasked with evaluating whether she is ready to return to the floor. Obviously, they know her medical history and understand the stakes better than anyone. But emotionally? It has to be complicated. They all care about Amy, and they all have professional responsibilities that could be affected if something goes wrong.

TOO SOON?
Michael, in particular, seems understandably concerned that Amy may be coming back too quickly. But Amy has never been someone who quietly sits on the sidelines and waits for permission to live her life. She wants her career and autonomy back. She wants proof that she is still capable of being the doctor she knows she can be.
The problem is that Amy’s confidence and Amy’s readiness are not necessarily the same thing.
That is what makes her return so compelling. We want Amy to succeed. We want her to get her life back. At the same time, there is real fear every time she experiences a flashback or a memory disruption while treating a patient.
The episode wisely refuses to frame Amy’s ambition as a flaw, but it also does not pretend that determination alone solves everything. Amy is healing, but healing is not linear. It is not neat. And it certainly is not easy.
The Interns Bring Chaos
A new group of interns arrives at Westside, and honestly, this is exactly the kind of fresh energy Doc needed entering the new season. New doctors always mean new personalities, new conflicts, new perspectives, and, of course, new people for Amy to terrify with her directness.
Amy and Sonya are placed in charge of the interns, and it is immediately clear that this group will test everyone’s patience.
Dr. Nicole DeSilva, the Stanford graduate, is smart, eager, and clearly determined to prove herself. But she also has a lot to learn about bedside manners. She jumps quickly into medical explanations and diagnoses without always recognizing that patients and their families are scared. Knowing the right answer is important, but knowing how to speak to someone in crisis matters just as much.
THE INTERESTING INTERN
That is something the episode handles well. Medicine is not only about being technically capable. It is about trust. It is about recognizing that patients do not hear “medical information” in a vacuum—they hear it while they are frightened, overwhelmed, and often imagining the worst possible outcome.
Dr. Caleb Ferguson, meanwhile, has a very different background. He is only 23, homeschooled, and already making his way through medical training. Dr. Caleb Ferguson has not had the most conventional path into medicine, and that could make him one of the most interesting new additions. He is clearly brilliant, but brilliance is not the same as experience. Watching him learn how to navigate the social, emotional, and unpredictable side of hospital life should be fascinating.
THE RICH KID
Then there is Dr. Graham, who arrives with a confident, almost aggressively competitive personality. He has clearly done his research on everyone around him, especially Nicole, because he wants to know exactly who he has to outshine. He comes from a wealthy family with deep ties to the hospital, and that immediately creates a Joan Larsen-esque energy around him.
There is a distinct difference between believing in yourself and believing you are better than everyone else. Graham seems to be leaning toward the latter.
THE SPEECH
Still, I will say this: characters like Graham can be incredibly fun on a show like Doc. They create tension. They challenge the people around them. And sometimes the most arrogant characters are the ones with the most room to grow.
Amy, though, is unexpectedly kind in her welcome speech. She tells the interns that they are there because Joan chose them before her death. The old Amy may have dismissed them, intimidated them, or made them feel like they had to earn every second of her attention. This Amy still expects excellence, but she seems more aware of how her words affect others.
Her growth matters.
The Case Is About More Than Seizures
The central medical case begins with Tori’s seizure during the school’s rehearsal for Romeo and Juliet. Soon, her teacher experiences symptoms. More students arrive at the hospital with dizziness, numbness, seizures, and other alarming reactions. As more people become sick, the doctors understandably begin looking for an environmental cause.
And honestly? Same. If several students and a teacher from the same school begin experiencing neurological symptoms, I would immediately be terrified of a gas leak, chemical exposure, contaminated food, mold, or something equally horrifying.
The episode does a strong job of building that anxiety. Parents are scared. Students are scared. The doctors are trying to find answers in real time. Everyone is waiting for the other shoe to drop.
Then the mystery takes a deeply human turn.

The students are not dealing with a traditional environmental illness. Their symptoms are connected to mass psychogenic illness—what the episode refers to as mass hysteria—triggered by fear, stress, and a shared traumatic experience.
The key is the school’s lockdown drill.
LOCKDOWN DRILLS
Tori reveals that a previous lockdown drill had been especially distressing. A student experienced a panic attack, the event went viral, and the school community absorbed that fear. Then, during the Romeo and Juliet rehearsal, the sound of a gunshot triggered those memories and anxieties. Tori’s underlying medical condition made her symptoms physically real, but the fear spreading through the school created a larger crisis.
This storyline is heartbreaking because it feels painfully believable. Children and teenagers are growing up in a world where active-shooter drills have become normalized. But as Gina points out, “normalized” does not mean harmless.
That line is one of the most important ideas in the episode.
Students may understand why lockdown drills exist. They may be told that these drills are for their safety. They may even be expected to treat them as routine. But being asked to imagine an active shooter entering your school is not emotionally neutral. It is frightening. It can create anxiety and trigger panic. And for some students, that fear does not simply disappear when the drill ends.
Amy’s handling of Tori in the end is surprisingly gentle. She explains that Tori’s condition was triggered by stress and that the experience may affect her moving forward, but she does not speak to her in the harsh, detached way old Amy might have. There is compassion there. There is patience.
That is not a small thing.
Amy may not remember the person she once was, but she is learning from the consequences of that person’s choices. She is actively trying not to repeat them. And that is one of the most moving parts of her journey.
Amy’s Past Is Still Coming for Her
The flashbacks to the day of Amy’s accident continue to reveal just how much was unraveling in her life before everything changed.
We see Sonya dealing with the fallout of Amy’s behavior and the complaints against her. It becomes increasingly clear that Amy’s professional reputation was not built on one isolated incident. She had a history of pushing people too far, dismissing concerns, and believing her skill gave her the right to operate without accountability.
Sonya’s anxiety is especially interesting because she has always had a complicated relationship with Amy. She has not always been the easiest character to connect with, but this episode gives us more insight into why she may have been so guarded or tense around Amy. She knew how difficult Amy could be and that behavior could have consequences for everyone around her.
And yet, seeing present-day Amy trying to become better makes their relationship more layered. Amy is no longer the exact person Mirella knew. But Mirella still remembers everything Amy did.
That is the tragedy of Amy’s amnesia: Amy is getting a second chance, but the people around her are still carrying the damage from the first version of her.
The flashbacks also continue to tease Amy’s complicated connection to Dr. Grant. Before the accident, Amy receives a call from him. We know she had some kind of memory connected to him last season, and there is clearly more to the story. The show is doing a good job of making us question what Grant knows, what Amy remembers, and what role he may have played in the events surrounding her accident.
Because the chemistry? The tension? The way he looks at Amy?
Something is absolutely going on.

Blair Underwood Is Here to Shake Things Up
Blair Underwood joining Doc as Dr. Richard Grant is a very exciting development. Dr. Grant arrives at Westside with confidence, charisma, and a reputation that immediately puts people on edge.
TJ meets him first, and it is clear that TJ is still trying to figure out where he fits in at the hospital. TJ has been through so much, and now that he has a new role at Westside, he is entering another period of professional growth. He is capable, he has real experience as a former combat medic, and he has the instincts of someone who has already handled life-or-death situations.
But Dr. Grant’s mentorship may not be simple.
COWBOY UP
Grant is described as a “cowboy,” which is never exactly reassuring when you are talking about a surgeon. He is bold, aggressive and willing to take risks. And he openly admits that he loves operating. He wants a larger surgical role at the hospital, and he is even willing to negotiate his pay if that is what it takes to make it happen.
The board also wants something from him: his research study. They offer him $3 million if he can bring it to Westside, and it looks like he may be willing to make that deal.
That could create huge consequences for the hospital.
Grant may be talented, but he also seems used to being the most important person in every room. That makes him exciting television—but it could make him dangerous in a hospital where the staff is already dealing with Amy’s return, leadership changes, new interns, and unresolved personal tension.
And yes, I need to address the obvious: Underwood smiling on screen is an event. Dr. Grant is suspicious, charming, intense, and very clearly paying attention to Amy. Gina notices. Amy notices. We notice.
A best friend always knows.
Amy and Jake Still Have My Heart
Amy and Jake’s relationship remains one of the most emotionally complicated parts of Doc. They are clearly still connected. They understand one another in ways other people do not. And even when they are awkward, distant, or avoiding their feelings, the chemistry is still there.
Jake is trying to find his footing outside of Amy’s orbit. He is spending time in cardiology and working with Margaret, who bluntly points out that he seems a little lost without Amy. It is a harsh thing to say, but it is not entirely untrue.
Jake has spent so much time tied to Amy professionally and personally that he now has to ask himself who he is when their relationship is no longer defining every part of his life.
REASONS
His decision to move away from pediatrics is also emotional. He admits that he began seeing his own child in his patients, and that is a completely understandable reason to need distance. Doctors are not robots. They are people who carry their own grief, fears, and histories into every room.
Meanwhile, Amy is doing her best to respect the boundaries she set last season. She understands that she and Jake cannot simply fall back into their old dynamic because it is familiar. But that does not mean the feelings are gone.
When they speak near the end of the episode, there is still so much unspoken between them. The way they look at each other, the way they understand each other’s jokes, the way Jake gently reminds Amy that she has always been a little too much—it is all there.
I will always be an Amy-and-Jake person. They have history, pain, trust, attraction, and the kind of complicated emotional connection that makes a television romance impossible to stop rooting for.
WHO COMES FIRST
But Amy also needs to find herself first.
Between Michael, Jake, Dr. Grant, and the ongoing question of who Amy wants to become, this season has the potential to give Amy one of the most compelling arcs on television. She does not need to return to the woman she used to be. She needs to become someone new—someone who is still brilliant, still ambitious, still unapologetically Amy, but also more compassionate, more open, and more aware of the impact she has on the people around her.
And honestly? I cannot wait to watch her get there.
I love that in the third season I am just as interested in Amy’s journey as I was when it started. She is so compelling to me and the show does a great job with that.
I can’t let go of Jake and Amy for all those same reasons but Ben and his connection with Amy intrigues me. I want to know what they did!
